Welcome! You have accessed the State Fund’s Threat Incident Reporting site. This site provides you with the ability to report incidents confidentially and securely and is hosted by Awareity, an independent third party, not part of State Compensation Insurance Fund.
Each and every State Fund employee has a vested interest and responsibility to ensure the safety of our workplace. You should bring any threatening behavior you encounter in, and around the workplace to the attention of your immediate supervisor. State Fund takes all threats seriously, but in order for each threat to be properly evaluated, a Personal Threat Report (PTR) must be filed. It is important that you, your supervisor, manager, and/or Program manager work together to complete and submit the report.
We also would like to encourage anyone in the general public to use this system to report incidents that involve a concern for the safety of employees at State Fund.
While anonymous reporting is an option, please be aware that should you wish to remain anonymous we cannot provide you a follow-up. Therefore, some form of contact information is beneficial.
